Fostair now for maintenance and reliever therapy in asthma

Fostair (formoterol/beclometasone) can now be prescribed for patients with asthma who require maintenance and reliever therapy (MART).

Fostair delivers beclometasone as extra-fine particles so doses delivered are not bioequivalent to most other beclometasone inhaler formulations.

MART should especially be considered for patients with uncontrolled asthma or with past exacerbations that have required medical attention. The recommended dosing regimen for Fostair in MART is one puff twice daily, with 1–2 additional puffs as needed up to a maximum of 8 puffs daily.

Evidence to support this regimen was provided by a 48-week parallel group study in 1701 patients with uncontrolled moderate to severe asthma. Patients were treated with the MART regimen or the standard maintenance regimen for Fostair (one puff twice daily plus concomitant salbutamol used as required).

The MART regimen significantly prolonged the time to first severe exacerbation and the rate of severe exacerbations compared with the standard regimen (p<0.001 for both). Patients receiving MART also achieved a clinically meaningful improvement in asthma control.
